If you have a scheduled telemedicine visit with our office, please review the Telemedicine Consent Form below. Your physician will verify your consent to telemedicine during your visit.

Once you have reviewed the Telemedicine Consent Form, you may click the corresponding link next to your physician’s name 5 minutes before your scheduled telemedicine visit time. Please note that if you are using a computer, only Google Chrome and Firefox are supported.
